Practice

Welcome to Richmond Medical Centre. We pride ourselves in the training of GP Registrars and Medical Students. The training provided for the registrar is tailored around the registrar's needs with support for assessment/nMRCGP. Support is given not only by your trainer but all members of the primary health care team. We offer professional training and supervision whilst providing a valuable, fulfilling and enjoyable experience. We tend to have 3 training doctors training with us at any one time - a mix of F2 trainee and GP Registrars.

Richmond Medical Centre is a purpose built premises, recently extended for training purposes, conveniently located 10 minutes from the centre of Sheffield. The Centre provides a warm, welcoming and comfortable environment for patients and staff. Car parking is available to the side of the building.

The Practice has approximately 7300 patients predominantly from Manor, Intake and Richmond, but we also have patients in Frecheville, Stradbroke and Handsworth. These areas are classed as medium deprivation with poverty and social fragmentation. The locality has particularly high levels of unemployment, currently standing at 14.5%.

We have a high elderly population. Young families are starting to move into the area. Of the 1116 patients who are 65 and over, 406 suffer from CHD. This is mirrored with other long term medical conditions. This particular patient profile will give the registrar an excellent training in managing patients with complex morbidity and needs.

The practice has a mixed social class 2-5 with few ethnic minority patients. However, some refugees and travellers have recently settled in the area.

The practice currently operates a nurse triage system for 4 hours per day Monday to Friday, except Thursday when clinics are 1 hour 20 minutes. This is staffed by our 3 nurse practitioners, one of whom prescribes.

Staff

Practice Manager
1 Full Time Nurse Practitioner
3 Part Time Nurse Practitioner,

5 P/T Receptionists. Two of the receptionists are trained as Medical Assistants, ie trained to take blood, do new patient checks, smoking cessation clinics and help with chronic disease management.

The practice has been at the forefront of developing the role of the practice nurse and as such we have three nurse practitioners that undertake triage.

All the nurses work along side the doctors having special responsibility for chronic disease management, screening, prevention and the running of various clinics.

Attached Staff

We have attached district nursing and health visitor teams. In addition there is a Midwife and graduate Mental Health Worker who attend the Practice. There is also a Counsellor and a Physiotherapist who sees NHS patients but will also see private patients. A private chiropodist also works from the Medical Centre.

Appointments

We run an appointment system. Each appointment is 10 minutes. Emergency appointments are booked through the nurse triage system. Registrar appointment times vary according to stage in training.

These triage appointments are mainly for children. The nurses take a history and make an assessment. The duty doctor may see the patient if required.

Out of Hours Care

Richmond Medical Centre has opted out of out of hours care, Sheffield GP Collaborative (now Local Care Direct) undertakes all out of hours work. GP Collaborative sessions are arranged with them to gain experience in out of hours care.

Computers

The practice is fully computerised and uses EMIS PCS system for prescribing, consulting, appointments, Pathology links and audit. We also use computers for accounting, and wages. We are connected to the NHS net and have a Web Page for patients. E-mail is available to you. GPs can connect to the system remotely.

Commitment to Quality

The practice achieves high QOF points and has well developed systems of clinical governance with regular meetings. GP registrars are encouraged to attend all our meetings.

The GP Registrar

You will be a valued member of our team. The practice is completing an extension and refurbishment March 2010 and you will have your own room with a computer and internet access. We have a well-stocked library. The first week or 2 in the practice will be taken up with an induction programme when you can spend time with the whole practice team. Each day differs but there is always Thursday afternoon free for the GPSTP half-day release scheme. Initially you will have 20 minute appointments aiming to reduce gradually to 10 minute appointments as you feel able.

Out of hours work is undertaken by the Sheffield GP Collaborative. Registrars gain out of hours experience by accompanying a trainer on shifts at the Collaborative, approximately twice per month. We do not operate a Saturday morning surgery.

All the partners, manager and nurses will take part in training. Tutorials are 2 hours a week. There is a daily debriefing session at lunch time.

Joint surgeries and videoing start early on. Although it sounds daunting, it is a great training tool. Regular debriefs occur with your trainer and the other partners in their absence. More recently a trial of debriefs between registrars and SHO has proven a valuable experience.

The 3 doctors in training in the practice are a valuable resource for each other. We believe that having this balance of trainers and doctors in training provides a stimulating but supportive learning environment.
