The Practice

Foxhill Medical Centre is an established practice on the North West edge of Sheffield.

The practice has a defined geographical boundary and accepts onto its list all applicants living in the area. Since we do not operate any screening of applicants to the list, we have frequently taken on "high demand" patients and families who, because of medical or social problems, have had difficulty being accepted elsewhere. Most patients live within one and a half miles of the practice.

The practice is committed to offering a high standard comprehensive service to the community. Most of these services have been available for some years. Services provided include:

Asthma, Diabetes, Hypertension Clinics
Well Women Clinics
Full contraceptive services
Integrated child health and developmental surveillance
Full immunisation/vaccination services
Heart disease risk screening
Life-style, smoking, weight management
Counselling/psychotherapy (group and individual)
Physiotherapy
Occupational health sessions
Massage therapist
Comprehensive maternity care, including intrapartum care - (we run one antenatal clinic weekly).

We offer complementary medical services such as massage, acupunture and herbalism and have access to Tai Chi and Yoga sessions.

Staff

Six General practitioners: Practice Manager, Receptionists, 3 Practice Nurses (2 whole-time equivalents). Occupational Health Workers, Specialist Health Development Nurse and Health Care Assistant, Physiotherapist, Counsellor, Herbalist, Administration workers.

It must be said that the employment of ancillary staff dos not significantly reduce the doctors’ workload. Rather it increases the range of services and therapeutic options available.

As a training practice we also offer high quality teaching to GP trainees, medical and nursing students.

The practice is approved for minor surgery, child surveillance.

The practice migrated to TPP System One in August 2010.

Demography

The practice is situated on the Foxhill estate and includes New Foxhill, Parson Cross, Southey Green, Grenoside and Chapeltown. Approximately 80% of the practice population live in council housing. Parson Cross is designated by Sheffield City Council as an area of poverty and Foxhill is one of five areas in the city designated as an area of acute poverty.

The area has a relatively high proportion of large families, single parents, and young children. The latest figures available to us show 45% not in employment. Debt is a big problem in the area.

The implication that health parameters are adversely affected by the social conditions of our patients is borne out by the morbidity data available and bears out our experience of our practice as one where demands and needs are high.

Practice Workload

List size: 6,100 patients
Age distribution: Average of 6% > 75 years, 8.5% < 5 years.
Birth-rate 2000: Approx. 100 new births
Consultation rate per patient/year: 2.9
Visit rate: 0.73 per patient/year
Out of hours visits: 0.21 per patient/year

On call will normally be arranged with your own trainer Dr Ian Nerurkar who does regular sessions at the GP Collaborative.

Practice Philosophy

The whole of the practice staff work together as a team in a collective spirit.

Practice policy is decided by all the practice workers collectively at the monthly practice meetings. A management committee comprised of representatives of the different worker groups in the practice oversee the running of the practice and its week by week management.

The practice recognises that ill health is related to socio-economic factors (class, employment, poverty, race etc) and that there is gross inequality in morbidity and healthcare. We recognise that there are political issues, which require political solutions, and that we are part of that process.

We support the concept of a NHS.

The practice aims to provide the best possible comprehensive care to the community it serves with easy access in a welcoming environment, and to provide that care in co-operation with individuals and families.

We are involved in all sorts of exciting projects varying from the community drugs project, H.N.A. of local community to Health and Fun days.

We are in the process of planning and developing LIFT premises which we hope will be up and running in the next 2years.

General practice is fundamentally a small business venture. Legally the doctors own the practice. There is an agreement to waive any rights to capital assets of the practice and not to treat the practice as a personal financial investment.
