Introduction
After an induction period which includes sitting in with all the staff you will initially have 20 minute appointments. During you whole time with us you have a nominated doctor to refer to at any time. As well as release for the VTS sessions, you will have feedback after each surgery, a weekly tutorial and a joint surgery with one of the trainers.
There has been a practice at Devonshire Green for about 100 years (with several generations of doctors!). The practice now operates from two purpose-built buildings. Devonshire Green Medical Centre (DGMC), situated close to the heart of the city, was opened in 1988; it is the main practice within the city centre. Hanover Medical Centre (HMC), serving Broomhall, was bought, refurbished and extended in 1993. In addition to the practice, Hanover Medical Centre is the base of HAST, Sheffield’s Homeless Assessment and Support Team, and of the attached Community Nursing Team (including a Case Manager and Community Matron).
We have around 7,000 registered patients and there are about 23,000 doctor consultations per year. Ours is an inner city population with high turnover (25% per annum). In addition to registered patients we care for homeless people who are roofless or in interim accommodation units and hostels. Homeless patients comprise about 1,500 of the annual doctor consultations. We have significant groups of patients from ethnic minority communities, including, for example, Somali refugees, Asylum seekers and families of overseas students. We have a much higher than expected number of patients with severe enduring mental health problems (approximately eight times the usual prevalence). We have a large number of patients who have abused or are abusing illicit drugs or alcohol. We also a large core of very long term local patients and have established very good relationships with the local community.
Work with the Homeless
The Singles Team of the Homeless Support and Assessment Team (HAST) is based at Hanover Medical Centre and includes staff from multiple agencies for the work with homeless patients. Since the project was set up in 1992, it has attracted national attention as a model for supporting health care of the homeless, and the practice received a Beacon award for its work.
Quality Standards
We score well in QOF and for patient access, having a combined drop-in and appointment system.
Computers
We use EMIS LV computer software, with paper light accreditation, computerised summaries, computerised appointments, repeat prescribing, disease registers, templates and active recall for chronic disease management. There is on-desk internet access in all consulting rooms and Prodigy. We have Path links, GP2GP, access to Choose and Book and ICE (the hospitals' results system). The practice has a library with computer terminal.
Locality
Sheffield is a great place to live and the practice is at the heart of the city! The beautiful Peak District National Park is only a 10-15 minute drive from the city centre and there are excellent and varied recreational facilities (theatres, concert hall, ice rinks, dry ski slope, Olympic swimming pool, to name a few). The city boasts a tram system and the leaner, cleaner steel industry is still very productive. There are two dynamic universities.... and there are apparently two football teams!
