Practice
We are a thriving and enthusiastic practice with a long history of GP training and teaching. The practice has been associated with the local vocational training schemes for 20 years. There are currently 3 trainers, one of whom is attached to the Barnsley scheme. Dr Amar Rughani is an Associate Postgraduate Dean in the Yorkshire and Humber Deanery and is an experienced RCGP examiner. Dr Mills is a Programme Director for the Sheffield GPSTP. The whole practice has a commitment to training.
In addition to the partners we currently employ 4 salaried doctors, 3 of whom previously worked as registrars in the practice.
We take pride in our organised approach to providing care to our large list of patients and we encourage innovation.
The emphasis in training is placed on real life general practice augmented by an evidence based approach, with the educational needs of the registrar being central to the whole process.

The administrative centre of the practice is Burncross Surgery, a purpose-built surgery, with 7 consulting rooms, spacious office and clinic areas, and ample room for expansion. The surgery was built in 1990. We also work from a refurbished purpose-built Health Centre at High Green. There are approximately 17,000 patients registered with the practice (2,500 per GP) which are split virtually equally between High Green and Burncross. Each doctor in the practice spends approximately half their time at each surgery.
Environment
The practice provides medical services for the villages of High Green and Chapeltown. Two other practices have surgeries in the district and there is overlap with neighbouring Practices on the periphery of the area. The population covers all social groups, with a preponderance of patients in skilled and semi-skilled occupations. The effects of social deprivation are not a major feature of our work. Housing consists of a mixture of privately owned property and local authority estates and flats. The area has a large concentration of young families and elderly people. The elderly are housed mainly in excellent warden-supervised accommodation. There is a 60-bed nursing home, predominantly for the care of the elderly mentally infirm and 3 residential/nursing homes for the elderly in the area. There is sheltered accommodation for 41 patients with special needs. The Practice is approximately 8 miles equidistant from Sheffield, Barnsley and Rotherham, is within 2 miles of the M1 motorway and is adjacent to open country and farmland. There remains little of local industry so that most people commute to work. There is a large comprehensive school on the edge of the area and a number of primary schools in the area.
Practice Administration
At present there are 40 staff employed by the practice including a general manager, office managers at each site, information technology manager, 7 members of the nursing team and a full supporting team of receptionists, secretaries, computer operators and clinic clerks. A chiropodist works 3 sessions per week and there are also 4 physiotherapy sessions a week. Counselling was introduced in March 1993 and provides 2 sessions per week. The practice is computerised with a fully integrated system linked to both surgeries with 47 terminals. We are paperlight and the computer system is used to control repeat prescribing, patient register, call/recall activity, consultations, home visits, patient histories and morbidity data.
We also employ 5 practice nurses with a triage role. Requests for same day appointments are triaged by the nursing team with the opportunity for them to manage a variety of minor illness using protocols.
Surgeries/Visits
Each doctor does 8 surgeries a week and each surgery lasts around 2.5 hours. Patients are currently booked at 10 minute intervals. Urgent visits and visits after 10:30am are covered by the on call doctor.
Care UK is used for all calls between 6:00pm and 8:00am and at weekends and bank holidays. One doctor currently does some OOH sessions for Care UK.
Registrar's Workload
Registrars are treated as genuinely supernumerary colleagues in our practice. Registrars' consulting rates will be determined by agreement and at a level that feels comfortable to optimise the educational benefits of the training period. A registrar towards the end of training would be aiming to consult at 10 minute intervals. Due to the nature of the practice population and the compact nature of the practice, the home visiting load is relatively light.
Clinics
The practice runs a full quota of clinics including general nursing, diabetic, asthma, hypertension, CHD, well person, family planning, ante-natal, well-baby, INR monitoring, earcare, continence, physiotherapy and counselling. The only ones the doctors are directly involved in are the family planning and well-baby clinics.
The Practice Team
In addition to the employed staff there are 20 other members of the community health care team either fully or partly attached to the practice.
