Filling in an HC1 form

An HC1 form is used to apply for help with health care costs. You can use this guide to help you to complete the form. Please contact Student Advice Centre if you need further help after following the guide below.

Part 1

  • Partner means someone that you live with as a couple. Do not fill in the section for your partner for a boyfriend/girlfriend that you do not live with.
  • You need to give your current address on the form. If you will be moving soon, you should give a correspondence address such as your parental home.  

Part 3

  • You do not need to give details of your housemates if you are a student.
  • You do not need to put down details of your children if you have already filled in Part 2.

Part 4

  • You need to give details of the money in all your bank/building society accounts.

Part 5

  • You should not use this section to give details of your student income/income from part time work.
  • You should provide details of any Child Tax Credits/Working Tax Credits in section 5.2.

Part 6

  • You should provide details of part time work in this section.
  • Don’t worry if you have more than one job or if your hours vary – put as much information as you can in Part 6 and explain your circumstances further if necessary in Part 9.

Part 7

  • All students should complete sections 7.1-7.4
  • Question 7.3 – if you have a joint contract tick yes and list your housemates.
  • Question 7.4 – if you pay rent, tick no and go to question 7.5
  • If you pay rent, provide details of your accommodation costs in Part 8.

Part 8

  • Sheffield University has semesters rather than terms. However you need to give term dates for this form. You should put the following dates on the form:
    • Term 1 starts at the beginning of Week 1 of Semester 1 and ends at the end of Week 12 before the Christmas vacation.
    • Term 2 starts at the beginning of Week 13 of Semester 1 and ends at the end of the week before the Easter vacation.
    • Term 3 starts at the beginning of the week after the Easter vacation and ends at the end of Week 15 of Semester 2 before the summer vacation.
  • If you receive funding for your course, you must include a copy of the award notification. If this is not in English, you need to provide a translation.
  • If you are not eligible to apply for any of the funding specified, don’t worry. You can explain this in Part 9 and give details of what you are living on.

Part 10

  • Make sure you have signed and dated the form before you send it off.

Evidence

  • Make sure you have provided all evidence asked for - proof of student income and part time earnings.
  • You can provide photocopies of any evidence requested.
