Spring Bank Playscheme 2012 - Wednesday 6 June to to Friday 8 June 2012

Playscheme is being held at King Edwards Lower School, Darwin Lane, Sheffield S10 5RG.

Booking opens on Wednesday 2 May and closes on Wednesday 30 May 2012.

Please submit your booking forms as early as possible as this will allow us to plan staffing and activities accordingly. As always, a full timetable of activities has been arranged from crafts, games, themes, trips and sports. All half day trips are booked and paid for on the day. A waiting list and rota system is in operation to ensure that all children attending the Scheme are able to go on at least one half day activity of their choice.

How to Book

• Decide which sessions you require (please refer to timetables for activities)
• Tick the sessions required on the application form 8.30am – 1.00pm & 1.00pm – 5.30pm
• Tick any early drop off/late pick ups required as must be booked in advance
• Note any medical conditions/allergies on the application form
• Sign all permissions on the application form
• If you complete the EYFS box, please inform your child’s setting about us as we may contact them
• Complete the total box and send the form, with payment (including any full day trips) to:
