This web-page provides information relating to the forms that our office can complete for students.

If you require any forms to be authorised by the University, you can either hand them in to our office, or you can email a copy. We are located at Financial Support Team, Ground Floor, Edgar Allen House, 241 Glossop Rd, Sheffield, S10 2GW (please note; during the summer of 2013, we may be moving offices so please check this web-page for up-to-date information before coming to the office). If you require an In-School Deferment Request Form to be completed, you can download a copy from the grey box on the right hand-side of this page. Please note that we cannot complete the form until you are a registered student.

You need to have completed Sections 1, 2 & 3 before we can complete section 4.

If you are a visiting student on a Study Abroad year, please take your forms to SSiD (Student Services information Desk) in the Students’ Union.

If you require a 1098-T Tax Form to be completed, we have a version of this form that we are allowed to complete, that confirms the amount of tuition fee you have paid. Please email usloans@sheffield.ac.uk for further information.

It can take between 3-5 working days for a form to be completed.
