Immunisation History and Medical Record
To process such a request requires 5 working days. After this time you will be able to collect the information from reception. If you require the information to be sent to your home address you will need to supply us with a stamped addressed envelope.
Please note that your medical records do not necessarily always contain details of vaccines that you had in school. If this is the case, we recommend you contact your school for this information.
If you require details of the vaccines that you have been given for either travel purposes or occupational health purposes, please contact us in one of the following ways:

- write to the Reception Manager, Mrs Gina Down at The University Health Centre, 53 Gell Street, Sheffield, S3 7QP
Medical Records
If you wish to view your medical record, please put your request in writing to Mrs Gina Down (Reception Manager) at the University Health Service, who will make arrangements with you for this.
Medical - Dental - Nursing - Students
We recommend that you contact the Medical Faculty´s Occupational Health Department for information regarding confirmed immunity to particular diseases eg hepatitis B and rubella.
