Are my records confidential?
| All consultations are confidential. Information will not be given to anyone without your express permission. However, please be aware that in order to provide you with the best clinical care, information may be passed to other healthcare staff involved in your care. |

| Please dial the usual University Health Service number on 0114 22 22 100. Outside of normal hours, your call will divert to the doctor’s Out of Hours Collaborative where you will be able to speak with a GP. This may not be one of the University Health Service doctors. |

Where can I take my prescription?
You can take your prescription to any pharmacy but the nearest one (Weldrick’s 27 61 321) is located below the University Health Service.

Are my prescriptions free?
| Prescriptions are free if you are a student aged 18 or under or are have a HC2 – Prescription Exemption Certificate available by completing a form HC1 from reception or the Benefits Office, if your income is below the prescribed level. |

Can I bring a friend into a consultation?
| You may bring a friend in with you to a consultation. Your clinician will advise you if this is not appropriate. |

Can I see a male/female GP?
| Yes, there is a list of the GPs on the Practice leaflet and at the link below which indicates whether they are male or female(UHS Staff Page). |

May I have a chaperone?
| Please ask the receptionist before your appointment who will arrange this for you. |

I need to see a Doctor today can I have an appt with Dr.....?
| We aim to offer an appointment with a doctor of your choice within 48 hrs or any other clinician on the same day. |

Could I see a nurse today?
| A Nurse Practitioner is available every day for the management and treatment of minor illness. She can be contacted directly for advice via the telephone option list (0114 22 22 100) between 8.45am and 10.00am. After this period, Receptionists will transfer patients to the Nurse Practitioner for advice. If the Nurse Practitioner thinks that an appointment with a doctor is necessary, this will be arranged. |

Could I make an appointment with a Doctor or Nurse?
| Patients may make an appointment with a doctor or nurse of their choosing. This may not be possible on the day of your choice. |

Can I make an appointment in advance?
| Advance appointments may be made online, by phone or at the reception desk. |

| All doctors have online appointments available. |

Where do I get a PIN Number?
| If you have not been issued with a PIN number during intro week, you will need to complete and sign a request form at the UHS reception. |

What kind of problems does the Nurse Practitioner deal with?
| The nurse practitioner is able to treat a wide range of minor illnesses, including sore throats, and is able to prescribe a wide range of medications as appropriate. |

Where is the chemist?
| There is a chemist/pharmacy located on Glossop Road below the Health Service. There is also a Boots Chemist on the corner of Glossop Road and Regent Street. |

What can the Walk-In Centre deal with and where is it ?
At all NHS Walk-In Centres you can receive healthcare information, advice and treatment for:
• Coughs, colds and flu-like symptoms
• Information on staying healthy/local services
• Skin complaints - rashes, sunburn, headlice, nappy rash
• Muscle and joint injuries - strains and sprains
• Stomach ache, indigestion, constipation, vomiting and diarrhoea
• Women’s health problems, eg thrush, menstrual advice
• Hayfever, bites and stings
Additional services at Sheffield Walk-In Centre:
• Ear syringing (patients will be assessed and booked into a later appointment)
• Treatment of minor infections (anti-biotics dispensing)
• General health care assessments
• Emergency contraception

Do you open on Saturdays?
| The University Health Service is not open on Saturday mornings. If you need to contact a doctor, your call will transfer to the doctor’s deputising Out of Hours Collaborative. |

What time do I need to call for an on the day appointment?
| Some appointments are released for on day booking, either online or by telephoning the University Health Service, on 22 22 100, as early as possible i.e. between 8.45am and 9.30 am. If your problem is urgent and all appointments are taken, please explain the nature of the urgency to the receptionist who will endeavour to fit you in with an appropriate clinician. This may not be possible with the doctor or nurse of your choice. |
